PROFESSIONAL CONDUCT - DN VERIFI CONSENT

The company is a member of DN Verifi and it is understood that the company will list you in its DN Hire list should you act unprofessionally by breaching the terms and conditions of your employment as defined by your employment contract and the company’s Standard Operating Procedures.

As part of your supply contract you must give your irrevocable consent to be included in this list and have read and accept the Terms of Use and Code of Conduct outlined at www.dnverifi.com.

Right to Appeal

You have the right to one appeal. On appeal you must provide DN Verifi with all supporting documentation to evidence your innocence and must accept its decision as full and final. It is understand that any listing under appeal will be classified as “Under Appeal” until verification of the facts by DN Verifi is complete.

Innocent

Should you be found innocent your listing will be removed with the names of all other entries made by the company from the DN Hire list and the company will be added to a DN Join list. You should note, that this does not affect listings made by other employers.

Guilty

Should you be found guilty, your entry in the DN Hire list will be highlighted to indicate “Verified” and permanently available to anyone to view online.

Inconclusive Supporting Documentation

Should neither the company nor you provide sufficient supporting documentation, DN Verfi may remove the listing at its discretion with no further action to the company.

Consent to put Information online

As part of your employment contract you must give your irrevocable consent that the following personal information relating to you will be stored by DN Verifi:

Employee Name, Address, Phone, Mobile, Email ID Type*, ID Number*, Photo (Upload), Start Date, End Date, Note, Reference Available (Y/N)

*ID Type and Number may be certificate of incorporation

As part of your employment contract you must give your irrevocable consent that the following personal information relating to you may be accessible online for others to view.

Employee Name, Photo, Note, Reference (Y/N), Status (Verified, Under Appeal)

You should note, that DN Verifi does not make public your address, phone number, mobile number, email or identification type/number and that only someone who has this personal information will be able to view your listing as defined above.
